Short summary
Gastric ultrasound (GUS) can assess the fasting state in trauma patients, with 54 patients to be included in an exploratory study evaluating gastric volume and emptying time approximately 6 hours post-meal.

Key points
- Trauma patients often cannot achieve a fasting state, increasing aspiration risk during anesthesia.
- Gastric ultrasound (GUS) is a novel tool to assess gastric content and infer fasting status.
- This study will include 54 trauma patients injured <6h after their last meal.
- The primary endpoint is the fasting state, with secondary endpoints including gastric volume and time since last meal.
- This is the first study to directly assess delayed gastric emptying in trauma patients.

Abstract
Problemstellung Trauma patients compromise a large proportion of the patients receiving emergency anesthesiologic care. Achieving a fasting state according to national or institutional guidelines by delaying surgery is often neither feasible nor desirable. Additionally, traumatic injuries may alter gastric emptying kinetics [ 1 ]. Increased aspiration risk due to residual gastric content needs to be weighed against risks associated with rapid-sequence induction (hypoxemia, difficult airway). POCUS assessment of gastric content to infer fasting state is a novel tool with promising reliability and applicability [ 2 ]. Multiple studies have reinforced its utility in the perioperative setting, but uncertainty remains in regards to trauma [ 3 ]. In these situations, identification of a non-fasting state can have profound impact on patient safety. Patienten und Methode Patients from 18 to 80 years, presenting to the local emergency department with a non-life-threatening traumatic injury, who ingested a meal <6h before the time of injury are eligible for inclusion. Exclusion criteria among others include any form of incapacitation, BMI>40 or concurrent medical condition known to impair gastric emptying. A total of 54 Patients will be included. In this single-center, exploratory, observational, cross-sectional study, fasting state of included patients is determined via gastric ultrasound examination, approximately 6 hours after the last meal. Ultrasound assessment of the gastric antrum is carried out as described here [ 4 ]. Ergebnis The primary endpoint is the fasting state of patients. Secondary endpoints include absolute gastric volume, time since last meal, injury. Schlussfolgerung To the best of our knowledge, there is currently no data available regarding directly assessed delayed gastric emptying in trauma patients. This study aims to generate new evidence regarding the fasting state and gastric-emptying time which could benefit patient care in the future.
