Key points

  • Malnutrition affects 40-80% of cancer patients, especially those with GI and upper respiratory tract tumors.
  • Nutritional status must be monitored closely with antineoplastic treatment, initiating support when needed.
  • Nutritional support options include counseling, oral solutions, enteral feeding, and total parenteral nutrition.
  • Patients post-stem cell transplant, post-surgery, with head and neck cancers, or on radiotherapy require detailed nutritional evaluation.

Abstract

Malnutrition occurs in 40 to 80 percent of cancer patients and is a major cause of morbidity. It is most commonly seen in gastrointestinal system tumors and upper respiratory tract tumors. Malnutrition status should be closely monitored from the onset of the disease in correlation with antineoplastic treatment and nutritional support should be initiated when necessary. This support may be in the form of nutritional counseling and use of oral feeding solution, enteral tube feeding or total parenteral nutrition. The best patients to consider for nutritional intervention are those with a potentially treatable disease or those likely to be disease-free for a long period after cancer treatment. Clinical situations where nutritional support will be evaluated in detail are patients after allogeneic stem cell transplantation, patients who have undergone surgery due to cancer, patients with head and neck cancers, and patients receiving radiotherapy.According to clinical nutrition guidelines in cancer patients, it should be assumed that the total daily energy requirement of patients is similar to healthy individuals and is generally between 25 and 30 kcal/kg/day. It is recommended that protein intake be above 1 g/kg/day, if possible up to 1.5 g/kg/day. Vitamins and minerals should be taken in amounts equal to the daily amount recommended for healthy individuals. Additionally, long-chain N-3 fatty acids or fish oil supplements can be used to stabilize lean body mass. As a pharmacological support, a limited period (1-3 weeks) of corticosteroid therapy or progestins to increase appetite may be considered, but caution should be exercised regarding side effects.
