Short summary

Effective management of knee pain and musculoskeletal disorders hinges on a deep understanding of the knee's complex anatomy and biomechanical principles, such as patellofemoral joint reaction forces.

Key points

  • The knee joint is one of the body's largest and most biomechanically complex structures.
  • Stability is achieved through a combination of bony surfaces, menisci, muscles, tendons, and ligaments.
  • The knee allows active flexion-extension from 0 to 140 degrees, incorporating rotational movements like the 'screw-home' mechanism.
  • The patella enhances quadriceps efficiency by balancing force direction.
  • Effective management of knee pain and disorders requires a thorough understanding of knee anatomy and biomechanics.

Abstract

The knee joint is the largest and biomechanically one of the most complex structures in the human body regarding articular surface and volume. Comprising the distal femur, proximal tibia, and patella, it consists of two interrelated components: the tibiofemoral and patellofemoral joints, which share a common synovial cavity. While the mechanical incongruence of the bony surfaces is corrected by the medial and lateral menisci, the stability of the knee relies heavily on surrounding muscles, tendons, and ligaments. Internally, the anterior and posterior cruciate ligaments maintain anteroposterior stability and restrict excessive rotation. Externally, the medial and lateral collateral ligaments act as primary passive stabilizers against valgus and varus stresses. Physically capable of active flexion-extension ranging from 0 to 140 degrees, the knee's kinematics also involve rotational movements, notably the "screw-home" mechanism. Furthermore, the patella functions to optimize extension efficiency by balancing the force direction of the quadriceps muscle. Ultimately, a comprehensive understanding of these detailed anatomical components and fundamental biomechanical principles, including patellofemoral joint reaction forces, is essential for the effective clinical management of knee pain and musculoskeletal disorders.
