Abstract
Metacognitive Therapy (MCT) is a transdiagnostic approach asserting that rigid and repetitive thinking patterns in response to negative thoughts and beliefs lie at the root of psychological problems. Differing from traditional Cognitive Behavioral Therapy (CBT), it targets the individual's process and style of thinking (metacognitions) rather than altering the content of thoughts. The "Cognitive Attentional Syndrome" (CAS), which includes worry, rumination, threat monitoring, and dysfunctional coping strategies, plays a critical role in maintaining psychological disorders. CAS results from an individual's faulty positive and negative metacognitive beliefs and chronifies negative emotional experiences. Clinical studies indicate that MCT achieves high recovery rates and is highly effective across various psychopathologies such as generalized anxiety disorder, depression, obsessive-compulsive disorder, and post-traumatic stress disorder. Techniques practically applicable in outpatient settings include Attention Training Technique (ATT), aiming at flexible attention management, and Detached Mindfulness (DM) training, enabling individuals to remain passive observers without engaging with thoughts. Through methods like the tiger instruction, blue giraffe experiment, cloud, and passenger train metaphors, patients are encouraged to shift from object mode to metacognitive mode and distance themselves from their thoughts.
