Prevalence of posterior root resorption in open bite patients undergoing clear aligner treatment: a single-arm retrospective CBCT study

Raghad Aljahdali, Hosam Baeshen, Ghassan Al-Turki, Fahad Alsulaimani

Short summary

Clear aligner treatment of anterior open bites in 27 patients caused significant posterior root volume loss, particularly in maxillary molars, with the palatal root of the maxillary left first molar showing the highest percentage loss.

Key points

  • Significant posterior root volume loss occurred in 27 patients with anterior open bites treated with clear aligners.
  • The palatal root of the maxillary left first molar showed the highest percentage root volume loss.
  • Volumetric assessment detected root resorption changes not consistently identified by linear measurements.
  • Root volume loss correlated with patient age and treatment duration, not posterior intrusion.

Abstract

Abstract This study aimed to quantify posterior root volume loss and assess its association with posterior intrusion. Linear root length changes were also evaluated and compared with three-dimensional measurements. This retrospective single-arm study included 27 patients with anterior open bites treated with clear aligners. Cone-beam computed tomography scans obtained before and after treatment were analyzed. Volumetric and linear root changes of maxillary posterior teeth were measured, and intrusion was assessed relative to the palatal plane. Significant volumetric reductions were observed in selected molar and premolar roots, with the highest percentage loss in the palatal root of the maxillary left first molar. Linear reduction was minimal and significant only in one premolar. Posterior intrusion was limited and not statistically significant. Root volume loss was not associated with intrusion but showed correlations with age and treatment duration. Volumetric assessment detected changes not consistently identified by linear measurements.
