Gizem Berfin Uluutku Bulutlar

Short summary

Over half of laparoscopic complications occur during abdominal entry (camera/port placement), leading to serious morbidity/mortality from bowel perforations and vascular injuries.

Key points

  • More than 50% of laparoscopic complications happen during abdominal entry (camera/port placement).
  • Bowel perforations and major vascular injuries are the most serious complications during entry.
  • Prior surgery, diverticulitis, and pelvic inflammatory disease increase complication risk.
  • Other complications include nerve injuries, infections, hernias, and issues from pneumoperitoneum or electrosurgery.

Abstract

Although laparoscopic surgery has characteristically low complication rates independent of the operation itself, managing these risks is of vital importance. More than half of the complications occur during abdominal entry, particularly during camera and port placements. Bowel perforations and major vascular injuries sustained during this stage rank among the leading causes of serious morbidity and mortality in the procedure. Factors such as a history of prior surgery, diverticulitis, and pelvic inflammatory disease increase these risks. Additionally, various complications may develop, including physiological effects due to gas insufflation during pneumoperitoneum formation, nerve injuries, surgical site infections, and long-term trocar site hernias. Electrosurgical errors during surgical dissection and hemostasis can lead to bleeding. In such cases, when bleeding control cannot be achieved or the patient's hemodynamic stability is compromised, rapidly converting to laparotomy (open surgery) is a life-saving decision. Furthermore, during pandemic periods such as COVID-19, preoperative testing and the use of specialized equipment are recommended against the risk of aerosol spread. Consequently, the correct application of laparoscopic techniques and the timely recognition and effective management of complications are fundamental requirements for surgical success.
